In order to hire a CMRA, you’ll need to require the USPS Form 1583. In order to complete this form you’ll need:
- Full legal name
- Physical address (no P.O. box)
- Two valid IDs (one photo ID)

The USPS Form 1583 is an added security measure that aids in the prevention of fraud and criminal abuse. It is essentially you authorizing the CMRA of your choice to accept mail on your behalf.

Acceptable Forms of Identification for USPS Form 1583
Forms of photo ID that may be used for the Form 1583 include:
